REPORTS AND DOCUMENTATION
Standard fire reports are usually available at the Hamden Fire Department Administrative Offices within 48 hours of the original call for service. Please note that a fire report records the basic call information and description of services, but is not a comprehensive fire investigation report. Fire investigation reports are only available through Hamden Fire Marshal’s office. *See report costs below.
Medical reports are considered Protected Health Information [PHI] and are covered under the Health Insurance Portability and Accountability Act of 1996 [HIPAA]. The Hamden Fire Department will release a medical report [PHI] directly to a patient that appears in-person with photo identification and signs an authorization for the disclosure or release of such information. *See report costs below.
For third parties representing a patient and requesting a medical report that documents treatment at a medical or emergency scene [PHI], a legally binding HIPAA release that authorizes the disclosure of PHI General Health Information / Drug / Alcohol / Psychiatric / and H.I.V. information, is also required. Private forms generated by third party legal representatives may also be accepted, however these forms and all other legal documents defining representation status may be reviewed by Town of Hamden counsel prior to release.
For HFD medical reports on incidents taking place after October 3, 2016 at 0800 hrs., a copy of any signed HIPAA release and documents defining representation will be scanned and electronically attached to the patient’s medical record.
There is a .50 cent-per-page administrative fee that must be paid for the release of any fire or medical report. This fee may be in exact-change cash, or by check made payable to: Treasurer, Town of Hamden.
